EU motion for the resolution covid-19 on the what 2020 has been agreed

Geneve, Switzerland 2020.

This COVID-19 infection knows no boundaries and neither should it know our aggregate reaction. It is just by cooperating on the response, in collaboration with all partners, and in support and under the initiative of the World Health Organisation (WHO) that we will win.

As we are living in remarkable occasions, the chance of keeping up a virtual World Health Assembly 2020 (WHA 2020) presents the unprecedented open door for Health Ministers of all WHO Member States to show their solidarity and their basic determination to beat the difficulties we as a whole face in reacting to this COVID-19 pandemic. Following the “period of silence” of all EU Member States, the EU motion for the Resolution COVID-19 on the World Health Assembly (WHA 2020) has been agreed. The EU proposition report has flowed to the other WHO Member States with a solicitation for support and co-sponsorship. The EU Ambassador officially introduced this in his address to the WHO Director-General, Executive Board and Member States during the WHO briefing.

The draft resolution tend to a scope of activities for the Member States, partners, non-state actors and the WHO-Secretariat in the general COVID-19 reaction. It likewise gives a chance to us all to salute the endeavours, far above what was required, of health professionals and frontline workers everywhere all over the place, just as to the WHO staff, for their devotion and skilful work for us all.

By actively participating in the preparation of this document, Croatia has contributed to the drafting of the EU motion for the Resolution COVID-19 and to impart a solid sign that the united EU is starting to lead in fighting the COVID-19 pandemic.

Risk of influence of covid-19 pandemic on the possible development of other epidemics

Although routine immunization for other pre-existing infectious diseases is generally maintained in most countries, there is evidence that COVID-19 virus adversely affects these activities. There is a reasonable risk that further outbreaks of other existing infectious diseases will override health systems already struggling with the effects of COVID-19.

“As the world strives to develop a new COVID-19 vaccine at record speed, we must not risk losing the fight to protect everyone and everywhere against other vaccine-preventable diseases,” said Dr Tedros, WHO Director-General.

Concerning the current situation of the COVID-19 pandemic, ongoing clinical research on five different vaccines for the COVID-19 virus. The worldwide pharmaceutical industry is focused on the invention and production of therapies and vaccines for COVID-19. Negotiations with regulators are ongoing on the possibility of reducing the legislation that needs to be respected by the time the vaccine is used. There are also problems in transferring vaccine production technology to other pharmaceutical centres, and in need of quality control of vaccine production.

At the same time, there is an increased risk that the COVID-19 pandemic will consequently affect the development of other epidemics. Research on nearly 20 million children worldwide indicates that more than 1 in 10 has missed additional measles, diphtheria, and tetanus vaccinations, and approximately 13 million children have never received these vaccines. Most of these children live in countries with already fragile health care systems, which limits their access to basic health services when they become ill. Measles remains a threat, especially if the immunization rate is reduced. Current projections indicate that as many as 800,000 people could be infected with the disease, and there is even greater concern about another resurgence, especially if the percentage of clefts is reduced by delays or suspension of planned immunization activities as a result of COVID-19.

Additional outbreaks of polio, diphtheria, and yellow fever are also of concern. Specifically, global coverage for Polio immunization is still far from the 95 per cent coverage needed to fully protect communities from a preventable epidemic of this disease. The future availability of vaccines with a longer period of immunization against the Poliovirus, for which research has been conducted before the onset of the current COVID-19 pandemic, is still out of reach.

Although worldwide supply availability has been largely maintained so far, the longer the COVID-19 pandemic lasts, the greater the risk of reaching critical supply levels. At the service level, however, potential supplies can be foreseen due to reduced supply or disrupted distribution. Furthermore, there are indications of diminished demand for vaccines in some countries due to lack of personal protective equipment for healthcare professionals, declining visits to health centres, psychological factors (eg fear of infection, false rumours) and lack of communication, coordination and specific community involvement on COVID-19. answer.

As the response to COVID-19 continues, health systems need to take additional action on immunization to reduce the possibility of outbreaks of other infectious diseases and new losses of life. These include the provision of emergency epidemic programs in places where services are disrupted, the provision of strong supply chains, disease surveillance and trained health professionals.

The new WHO Immunization Guidelines and COVID-19 recommend that temporary immunization campaigns be suspended where there is no active epidemic of a vaccine-preventable disease. However, it is urged that priority be given to continued routine immunization of children, as well as adult vaccination, as in the case of influenza, for the groups at highest risk. If immunization activities are to be discontinued, vaccination should be compensated as soon as possible, favouring those at the highest risk.

Hope for a common solidarity future of health for all

At the proposal of the European Union, the 73rd World Health Assembly (WHA) adopted the historic Resolution COVID-19, which was co-sponsored by as many as 146 WHO member states.

From the beginning of the drafting process until the adoption of the resolution, Croatia actively participated in the harmonization of the content of document with other WHO member states. By adopting the COVID19 Resolution, the world symbolically showed unity in the fight against the COVID-19 pandemic.

The process of harmonizing the text of the Resolution was more than challenging. Namely, the growing geopolitical tensions between the United States and China gave an uncomfortable feeling of accepting the risk of a negative outcome.

This process of harmonizing the views of WHO member states was characterized by the usual separation between countries that were more protective of their pharmaceutical industry and those member stares, aided by civil society groups, who wanted to break patent monopolies over critical health products.

The COVID-19 resolution, with its content, includes proposals for all important items of the common position and coordinated activities of EU countries related to the COVID-19 pandemic: recognition of health workers and everyone on the front lines, especially women; support for the leading role of the WHO and the UN; solidarity; multilateralism, the maintenance of essential health services during a pandemic; EU funding assistance; the equal right of all to medical treatment, therapy and vaccines and a transparent evaluation of everything done during the fight against the COVID19 pandemic.

The COVID-19 Resolution provides the basis for the timely resolution of the common struggle of humanity against all future threats of a similar nature.

By jointly proposing this COVID-19 Resolution, the EU has, above all, demonstrated its unity. Encouraging the unique international cooperation of all the people of the world, the EU has presented itself as an important global player in this challenging period for humanity.

How much are we suited for changes?

Turning a new career page is an invaluable experience. Bravery, risk and experience are the most important – it is the motto of life. Every change brings something new, especially in this year of life anniversaries. But experience, knowledge and empathy for the needy are the fundamentals of the medical profession and quality health service.

Although my beginning in 2020 was marked by the publication of a five-year work of an international group of top experts in orthopedics and traumatology in the KSSTA journal, in the preparation of which I personally participated in “Management of traumatic meniscus tears. The 2019 ESSKA Meniscus Consensus” new life opportunities opened the door.

This was indicated by the parallel publication of my first scientific paper in the field of social sciences, diplomacy and public health „International Medical Knowledge Transfer as a Tool of Public Diplomacy: The Case of Croatia“ in the journal Croatian Political Science Review.

The World Health Organization and its center in Geneva, Switzerland was a new challenge.
The sudden onset of the global COVID-19 pandemic and the extremely extensive engagement in the work of the WHO EU Coordination of Member States soon showed that my decision was the right one.

However, when colleagues from the International Institute for Health Impact – iifhi.com asked me to participate in a project to spread knowledge about news from my medical profession, I readily responded to the challenge. The result of our collaboration is a dozen IIFHI educational video clips.

Through lifelong education, dissemination and application of newly acquired knowledge in connection with modern methods of treatment, we enrich medicine in the most vital, professional part of its implementation.

With the dissolution of the convocation of the Croatian Parliament, my role as an external member of the Committee on Health and Social Policy of the Croatian Parliament ceased. I was elected as an external member of the Committee on the proposal of my Croatian Diplomatic Club and support from the members of parliament from the north of Croatia.

The meetings of the Committee were filled with interesting discussions, especially thematic ones, which were very often related to current topics and issues in Croatian healthcare sistem: related to drug use, tobacco use, mandatory vaccination program, obesity awareness, public health and food safety, child psychiatry, the issue of nurses, the availability of therapy and treatment of rare diseases, autism, the fight against cancer and the creation of an oncology registry, international humanitarian law, the organization of emergency helicopter service in Croatia, and many others.

Of particular interest was the session of the Committee, which discussed the impact of the shortage of doctors on the perspective of the functional sustainability of the health system. Namely, in the discussion on this topic, a realistic presentation of the unpleasant situation in Croatian health care was obtained. Namely, the basis of problems and mutual disputes is the clash of highly set criteria of legislation, patients’ rights and imperatives of quality of medical services on the one hand, and the number and rights of doctors and health personnel, organization and financing of health services on the other. Aware of the real facts and the state of the Croatian health care system, the members of the parliamentary committee, which consists mostly of doctors, expressed their views in support of changes in the system of negotiating the rights of doctors and improving the organization of their work.

It is worth mentioning that at one of the last sessions of the Committee, back in February this year, we had the opportunity to discuss the readiness and organization of the Croatian health system to respond to the COVID19 pandemic. The actuallity of the topic, the importance of the timely reaction of the Committee and the knowledge that the then interlocutors are today members of the Croatian Crisis Staff who stand behind the excellent Croatian result in the fight against the pandemic, give me additional conviction that the past work of the Committee was quality.

This is the end of an extremely interesting four-year mandate of socializing with fellow doctors, nurses and representatives of the health and social profession in the Republic of Croatia.
Most of us, after a certain period of life, especially in a career, almost always have the impression that more could have been done. But the experience of working in a parliamentary environment, above all the opportunity to freely express my ideas, participate in debates, make decisions and make legislative proposals, leaves an immense wealth for the further course of my career.

Are there new options for COVID-19 therapy?

The history of medicine is saturated with examples of drugs that have worked in the lab but have not acted on a living human body or were actually harmful. World Health Organization President Dr. Tedros has urged individuals and countries to refrain from using therapies that have not been shown to be effective in treating COVID-19.

The possibility of using reconvalescent blood plasma as a potential drug for COVID-19 infected patients is being explored. Namely, patients who have recovered from COVID-19 in the blood have antibodies that can be effective against infection. These antibodies in blood plasma can be administered to COVID-19 patients to stimulate the host immune system in its own response.Cloned antibodies from recovered COVID-19 patients showed impressive ability to neutralize SARS-CoV-2, a cell-borne virus, and reduced the percentage of live cell-binding virus by almost 100%

Covid-19 clinical trial

Early clinical trials in China on 10 patients showed a “significant improvement” within 1 to 3 days after receiving blood plasma transfusions, with 2 of 3 patients discarded from mechanical ventilation shortly after transfusion. Chinese experts have based their knowledge on limited success in treating other coronaviruses, such as SARS and MERS.

A patient who is recovering with a previously confirmed laboratory diagnosis of COVID19 can donate blood plasma only 2 weeks after the symptoms have resolved, and only if he is then tested with a negative result for COVID-19. Blood donation thus donated is given to a patient with severe COVID-19 disease after undergoing other intended laboratory infusion testing.

Although a promising type of therapy, reconvalescent blood plasma is not effective for every disease, including viral diseases such as Ebola. Success in cell culture testing has yet to be demonstrated on a living human organism.

I attended the truly excellent World Health Summit in Berlin, Germany, which brought together numerous health professionals, WHO representatives and health ministers from around the world.

World health leaders have discussed the global availability of modern medical technology and knowledge and the simultaneous problem of unavailability of healthcare at the local level.

A very inspirational speech was delivered by German Health Minister Jens Spahn, announcing that the global digitalization of health would be a focus of interest during their EU presidency in the second half of 2020. Also of interest was the approval by the auditorium of its confirmation of the introduction of the mandatory vaccination against measles in Germany.

Such a firm stance is crucial, especially today, both in Croatia and globally, to successfully implement prevention and eradicate infectious diseases.

Through a mutual discussion on global health priorities, I presented to several WHO members the Croatian EU Presidency, which is happening next year, and the tradition of prof. Andrija Stampar at the WHO, believing that this kind of networking is imperative for promising positive cooperation.

At the ERAS (Enhanced Recovery After Surgery) symposium of the Croatian Society for Accelerated Recovery after Surgical Treatment in Vukovar, I presented the ERAS project and work experience with patients at my Department of Traumatology and Osteoarticular Surgery, University Hospital Center Zagreb.

The symposium was an opportunity to discuss the current situation about the implementation of ERAS protocols and spread the idea of the importance of multimodal and multidisciplinary approach in the treatment of operated patients in all surgical professions in the Republic of Croatia and the world. I have written about the ERAS protocol in orthopedics and traumatology in previous blogs.

During my presentation of our experiences, I gave special attention to the challenges and problems we encounter when organizing, leading the ERAS team and implementing such a protocol in today’s specific working conditions within the Croatian health system.

A new ERAS project in bone and joint surgery is being prepared, which we will work on together with our Slovenian colleagues from the University Clinical Center in Ljubljana.

I visited Slavonia and Baranja with most of the members of my ERAS team in bone and joint surgery at the University Hospital Center Zagreb. I used the visit for the purpose of the so-called “Team building” in order to raise the quality of intercollegiate cooperation and satisfaction.

Recently, Croatian National Television broadcast „Dijagnoza“ visited my ERAS (Enhanced Recovery After Surgery) team in the Clinical Hospital Zagreb. With a live talk about the new features of our treatment in the bone and joint surgery, the broadcast was focused at the new possibilities of ERAS treatment in Orthopedics and Traumatology.

Patients who underwent hip fracture operations have higher morbidity and mortality rates and a higher prevalence of delirium than the national average.

ERAS is a multimodal, multidisciplinary approach to the care of the surgical patient. Enhanced Recovery After Surgery process implementation at my Department for Traumatology, Bone and Joint Surgery at the Clinical Hospital Zagreb involves a team consisting of orthopedic and trauma surgeon, anesthetist, psychiatrist, radiologist, endocrinologist, nutritionist, physiatrist and physiotherapist, nurses and social worker.

Implementation of ERAS programs results in major evidence-based care improvements in clinical outcomes and cost, making ERAS an important example of value-based care applied to surgery. Enhanced Recovery After Surgery approach for hip fracture operations allows patients to return home faster and get back to normal activities sooner. When used in such high-risk surgical patients, a modified enhanced care recovery approach lowers mortality rates, reduces hospital stays, and results in fewer complications following hip fracture operations. Audit of process compliance and patient outcomes are important features.

Department for Traumatology, Bone and Joint Surgery at the Clinical Hospital Zagreb started a project with ERAS implementation in operated patients for hip fracture. Preliminary results confirmed our hypothesis about the over all improvement in the surgical treatment outcome of patients who underwent hip fracture operations.

This week I am participating at the UN Summer Academy in the UN Centar in Bonn, Germany.

The UN Academy provides an exclusive opportunity for a dialog with professionals and thought leaders in Health and other systems who are currently working towards the implementation of the 2030 Agenda and the Paris Agreement on a daily basis.

The Academy combines academic rigour with cutting-edge thinking, builds on renowned faculty and is open to professionals from Health and other sectors. Among 50 selected representatives from all over the world I have a privilege and opportunity for interactive learning and networking with the most qualitative UN Health professionals.

WHO information about achieved global results of UN Agenda 2030 are very interesting. Despite huge financial investments during the recent years, only partial improvement was achieved towards the goal of reducing the percentages of neonatal death and number of Malaria and Tuberculosis infected population, while the percentages of HIV infected population increased.

Simultaneously, the number of deaths caused by injuries from traffic accidents increased significantly. According to these statistics, the problem of proper possibilities for medical treatment of traffic injuries reached far beyond borders of the standard health issue and it encaptures other fields of society’s spheres (car industry, traffic and road construction etc).

The most significant information is that the continuous progression of the lack of health professionals’ is spreading all over the world. This leads to unequal access to health services, even in some regions of wealthy and highly developed countries. Having the similar situation in Croatia, it seems as the worst scenario of “closed circle”, where the natural migration of the domestic health workforce leads to unsustainable functioning of national health systems, which is the case in many countries.

Upon successfully completing this highly demanding and positively fulfilled working week in UN I will try to transfer new knowledge and experience to my Croatian colleagues and use it for my work in Croatian health diplomacy
